Underneath your own name at the top of the form there is no need to add a name if you are happy with the Chairman as your proxy (Jim Glennon - great chairman)
Below that I ticked "In favour" for everything.
The three going for re-election are superb. The special resolution being voted for in item 4 is:
“7 – If upon the winding up or dissolution of the Company there remains, after the satisfaction of all debts and liabilities, any property whatsoever, the same shall be paid to or distributed amongst the Members of the Company at the time of such winding up or dissolution, on an equal basis”
(The 'members of the company' are all the GP members of Medisec)
