    IMO UPDATE ON COVID VACCINATION PROGRAMME

Friday 7th May 2021

Thanks to the work of GPs and their Practice Teams around the
country the vaccination programme for Over 70s and now Cohort 4
and 7 has been a huge success with GPs delivering 50% of the
vaccines administered up to end of last week.
Notwithstanding the early logistical issues and new
recommendations from NIAC, General Practice has proven once again
the value of the GP/Patient relationship and the ability to
deliver a complex vaccination programme efficiently.
GPs are now confirming and scheduling clinics for Cohorts 4s and
7s and the programme nationally will now move to an age based
only programme.

Vaccination of Pregnant Women

The vaccination pathway for pregnant women is being finalised and
will beavailable early next week. This will be arranged through
the maternity hospital setting and GPs are not being asked to
undertake vaccination of this group of patients under this
programme.

Vaccination of Cohort 4 and 7 in General Practice

The HSE have confirmed that the current arrangements in respect
of Over 70s, Cohort 4 and 7 remain in place and GPs are advised
to:

GPs continue to identify and vaccinate patients aged under 60
(those patients aged 50-59 are eligible for this programme)
In relation to Cohort 7 the objective is to ensure all High Risk
patients are vaccinated as a priority and in this regard the
clinical guidelines are an aid to identifying patients but at all
times your clinical judgement will prevail in terms of
identifying those patients most at risk from serious illness or
death as a result of COVID.
Where you have given indicative numbers for Cohort 7 to GP
Vaccines please note it is possible, within the overall supply,
to increase your order if required and such requests should be
directed through your RM or GPvaccines.
GP may vaccinate patients aged 16-18 with Pfizer (Category 4 and
7) where they have not been vaccinated in the acute setting. It
should be noted the vast majority of these patients have already
been vaccinated within acute or other community setting.
Vaccination schedules are to continue i.e., Dose 1 and Dose 2 for
the above programmes and there is no requirement to determine the
COVID status of patients within this programme.

Secure Online Access to Check Vaccination Status of Patients

As advised previously, secure online access via Healthmail will
be available from Monday 10th May 2021 to allow GPs to check the
vaccination status of patients. It is recognized that such access
is critical in terms of the organisation of clinics and should
have been in place prior to the commencement of this programme.
Support information and videos will issue with the rollout.

Referral Pathway for Patients to Receive 2nd Dose Where GP
Administered Dose 1 from Surplus and is not continuing with
programme

In circumstances where GPs provided Dose 1 vaccine from excess
vaccines at clinic to patients, but the GP is not continuing with
the programme a referral pathway will be available via Healthlink
from Thursday 13th May to allow patients to receive Dose 2.

The referral process will require:

Patient name, address, contact details, PPSN, (there is an
Eircode facility which is automated once you type in address)
type of vaccine (Pfizer or Moderna) and date vaccine was
administered. Once the referral is made it will be a matter for
the HSE to contact the patient to arrange for Dose 2 in line with
the interval period.

The guidance in respect of the use of excess vaccines remains:

GPs continuing with cohort 4 and 7
For GP’s vaccinating cohorts 4 and 7 where spare vaccines arise
you may use clinical judgment in vaccinating patients aged 18-59
who are at risk of serious illness and on your contingency list,
prioritising in sequence your older patients first.

GPs NOT continuing with cohort 4 and 7
GP may use any spare vaccines from final 2nd dose Over 70s
clinics to administer 1st doses to patients in cohort 4 and 7 and
use referral pathway detailed above to refer for Dose 2.

Referral Pathway for GPs who are not participating in Cohort 4
and 7 Vaccination Programme

Where GPs are not in a position to continue with the vaccination
programme for Cohort 4 and 7 a referral pathway for those
patients via Healthlink will be available from Thursday 13th May.
Support information and videos will issue with the rollout.

Once referred the HSE will register patient and invite to a
vaccine appointment in one of the HSE Vaccine Clinics.

The referral will require:

Patient name, address, contact details, PPSN, (there is an

Cohort (eg 4 or 7) as per clinical guidelines.

This pathway is an option for GPs to refer patients in Cohort 4
and 7 bearing in mind:

Patients aged under 50 do not require a referral as this age
cohort are being asked to register on the national portal for
vaccination.
Patients are not referred for any specific vaccine and will
receive the vaccine available at the setting they attend, or the
vaccine advised for their age cohort
All citizens will be invited to register via the national portal
in the coming months in line with their age cohort.

GPs who wish to continue with Age Based Vaccine Clinics

In terms of the wider vaccine roll out over the coming months we
are in discussions with the HSE to facilitate those GPs who wish
to continue with an age-based vaccination programme using Pfizer.
It is recognized that this will not be feasible for all practices
bearing in mind the need to facilitate the normal important work
within General Practice. More points of vaccination will be
available to patients over the coming months so at all times
there will be alternative pathways for your patients.

When details are available, we will communicate further and
holding a webinar following which practices will determine
whether or not they will to continue.

Reminder: GP Teams 2nd Dose Vaccination

For GPs and GP Team Members who were vaccinated at the GP Clinic
on 14th February please note you will be contacted by the HSE in
relation to date/location for dose 2 vaccines in line with the
new recommended interval periods for AZ – those being 12 weeks
for persons over 60 and 16 weeks for patients aged under 60. You
will receive a communication offering a vaccine appointment at a
HSE Vaccination Centre next week for those over 60.

Reminder: Vaccines for New GP Team Members

Where practices have recruited additional staff (Cohort 2) since
the Vaccination Clinics please see below a list of contact
details per county to arrange for such staff to be vaccinated.
