Hello all!

I have a patient recently intervened  gastric bypass and needs iv iron as oral iron wont do. I have been searching around how to manage this as we don’t do these procedures in our practice for several reasons: no space , inadequate facility, short staffing etc. It does not seem appropriate to me involve a consultant for this as per my point of view is pretty much straight forward procedure , but the location and logistic is an issue right now

Any idea where could I send this patient ? She is private, Hgb is 12, abs serum iron is 10.

Thanks for the advise

Hi Jose
I dont think any of our practices do this although it has come up here before. I think there could be indemnity issues for us to provide in GP. It might be possible that if she has insurance a local private GI consultant might get her in to have it done. Alternately examining if she definitely needs it- would she absorb oral iron (Hb looks ok)?
Just my thoughts!
Best regards
Jean

Hi all,
In the past when we have had patients who can not tolerate oral iron
replacement, gastroenterology in TUH have brought them into the day ward
following OPD assessment for iron infusions.
If she has private cover, it should in theory be easier to get an
appointment with gastro.

A long time ago, I used to give im iron but this has gone out of favour for
various reasons so don't offer this option. Are people still given im iron?
