She is carrying out a study examining some aspects of Chronic Disease Management from a wider perspective than just the HSE program.  She looks at diabetes, atrial fibrillation and heart failure.
She needs our help and I’m sure we would all be willing to help, especially as it is very easy for our practices to participate.  In a nutshell she needs us to fill out a consent form plus a brief survey about our practice and then extract a few iPCRN reports from our Socrates program.
Your practice manager will be circulated with details in the coming days, but I attach everything necessary to complete your practices participation if you want to go ahead and partake now.  You can ask your practice manager to scan and email the completed forms and to do the data extraction if you wish. The consent and questionnaire are best filled up by one of the practice doctors.
All you have to do is complete the consent form and the questionnaire, scan them back in and e-mail them to me – mjoyce@centrichealthblessington.iemailto:mjoyce@centrichealthblessington.ie DO NOT USE REPLY TO THIS E-MAIL.
Then either do the data extraction yourself or ask your practice manager to do it and forward the resulting 3 e-mails to me, remember to include the name your practice.  No patient information whatsoever appears in the reports only aggregated data.
Otherwise, we will be in touch in the coming weeks, Jo Cassidy and myself are helping out.  If you are asked for consent, I’d be delighted if you would agree to take part.  We can of course supply you with all the details including ethics approval etc.  if you need to see them.  It would be great to see all practices participate in support of this commendable Centric academic sponsorship.

PS: it’s not a competition and I’m sure there is some overlap but, two groups of practices are partaking in the study, ourselves in Centric and the UCD attached teaching practices.  Let’s make sure our return rates beat theirs.
